Ptosis repair

Hi, I am seeking a non-Kaiser recommendation for a surgeon to perform a third ptosis repair on one eyelid (was born with it) and correct the scar left by the second, mostly unsuccessful one. Would be very grateful for recent recommendations and a sense of pricing if you paid out of pocket. 

I am specifically seeking to have it done under sedation, not just local anaesthetic while fully awake. That was a very bad experience last time.
