School Volunteer TB Risk Assessment Form through Kaiser

I'm registering to be a volunteer with Alameda Unified and the the volunteer level I need (to drive for high school sports) requires a TB clearance. I have no risk so don't actually need a TB test and just need the CDPH form filled out by a health care provider saying I have no risk factors. I'm a Kaiser member and know that getting any form from them is an absolute nightmare right now. Anyone do this recently and have any tips?

I have to do this periodically for work. By far the quickest way to get it done is to just bring the form along on a routine appointment. But the new online system is also a big improvement over the old system where you had to send everything via email, at least in my limited experience. Note that if you haven't been seen your GP in the last 12 months, they won't do the forms online anyway, so then you'll have to go in. If you've had a physical in that window, it's worth trying the online submission first. 

I just emailed my Kaiser GP about this exact thing. She referred me to the Release of Medical Information (ROMI) office, which also handles routine medical forms. I submitted the form through their online portal. They said I would get it back within 3-15 business days. (Ha! Pretty big window there.)

It's super easy, no appointment necessary - I just renewed my TB clearance for school volunteering last year. Start at the KP Release of Medical Information (ROMI) site: https://mydoctor.kaiserpermanente.org/ncal/health-guide/release-of-medical-information. Choose the "Medical Form Request" option and complete the online request for medical records and indicate which form(s) you need, and they'll notify your doctor to complete the request. If I remember correctly, they sent me the default Kaiser form and not the exact form the school gave me, but the school accepted the Kaiser form. You can also use the ROMI system to get sports physical forms filled out for school or camp, in case that helps anyone!

Super easy. Just go to your KP.org account and click on “release of information”. You can upload your form and the secretaries will forward it to your physician for signature.  I believe you can fill out the questionnaire yourself before you upload it. If your doctor has questions, they will contact you. 

I did it a few years ago so it may have changed now but I brought my form to the front desk at Kaiser Alameda and they just brought me to the back and gave me a TB test. Wouldn’t hurt to walk in and ask what the procedure is. The people actually working there will know better than the phone tree if you call.
